An AFP test is primarily used to determine the level of alpha-fetoprotein (AFP) in a pregnant woman's blood. The test determines the baby's risk of developing certain genetic disorders and birth defects. AFP tests are typically performed between 15 and 20 weeks of pregnancy. 

AFP is a protein produced by a developing baby. AFP normally passes from the baby into the pregnant woman's blood. Certain circumstances can cause a baby's body to produce more or less AFP. If your AFP blood levels are higher or lower than normal during pregnancy, it could mean that: 

  • The baby is at high risk of having a genetic disorder, such as a neural tube defect, which is a serious condition that causes abnormal brain and/or spine development in a developing baby.
    • Down syndrome is a genetic disorder that causes mental retardation and other health issues.
    • Your estimated due date is incorrect. Because AFP levels rise and fall at regular intervals during pregnancy, an abnormal AFP level may indicate that your baby is due sooner or later than expected. This is the most common cause of elevated AFP levels.
  • You're expecting more than one child. Because each baby produces AFP, your AFP blood levels will be higher if you have two or more babies.
  • Other names for AFP Maternal include Maternal Serum AFP and msAFP screen. 

What is its purpose?

During pregnancy, an AFP blood test is used to assess the baby's risk of birth defects and genetic disorders such as neural tube defects or Down syndrome. The test does not make any medical diagnoses. If your AFP test results are abnormal, it indicates that additional testing is required to determine whether your baby has a health problem. 

An AFP test may be used to help diagnose certain cancers that cause high AFP levels in adults in people who are not pregnant. When used in this manner, the test is known as an AFP tumour marker test. 

Why do I require an AFP test?

If you are pregnant, an AFP test is usually offered between the 15th and 20th week. Your doctor may particularly recommend the test if you: 

  • Have a history of birth defects in your family?
  • Are 35 years of age or older
  • Do you have diabetes?
  • Have you used any medications or drugs while pregnant that could harm the baby?
