This test determines the level of free thyroxine (FT4) in your blood.
T4 is one of the thyroid gland's two major hormones (the other is called triiodothyronine, or T3).
T4 accounts for nearly all of what we call thyroid hormone, while T3 accounts for less than 10%. Thyroid hormones aid in the regulation of the body's metabolism (that is, how the body functions).
The majority of T4 in blood is bound to a protein; less than 1% is free. Because this is the biologically relevant fraction, the blood test measures the amount of free (unattached) T4 hormone in your blood.
The pituitary gland, which produces TSH, and the thyroid have a feedback system. TSH produced in the pituitary gland normally drives thyroid T3 and T4 production. TSH levels rise when thyroid hormone production falls, and TSH levels fall when T3 and T4 levels rise.
