HBeAg is the "soluble" or extractable component of the hepatitis B virus's "core" antigen (HBV). The presence of HBeAg indicates that the virus is active and highly contagious. HBeAg is detected in the bloodstream shortly after HBsAg. It remains in the blood for approximately 3 to 6 weeks after the acute (symptomatic) phase of the disease and may persist for an extended period of time or reappear in the case of active chronic hepatitis. As a result, the HBeAg test is most commonly used to determine the level of contagiousness. It can also be used to assess the efficacy of a treatment. A positive (reactive) HBeAg result combined with a negative (non-reactive) anti-HBe result confirms the presence of a high level of contagiousness. When the anti-HBe is also reactive, it indicates that the individual is healing and is less contagious.

A non-reactive HBeAg result (with a reactive anti-HBe result) in a person with hepatitis B indicates that the infection is in its early stages, before the virus reaches peak replication. If the anti-HBe test is positive, it could mean that viral replication is now very low. There are HBV subtypes that do not produce HBe. These subtypes are typically found in Asia, the Middle East, and, to a lesser extent, Europe.
