Sputum cytology is the process of examining phlegm or lung secretions for malignant cells. A sample of mucus is collected from the patient's cough and examined under a microscope to look for any signs of malignancy. Samples are frequently taken every morning for a few days.

Sputum cytology: What is it?

Sputum cytology is a quick, low-cost test, but it's not always reliable. Even though the rate of false positives is modest (1%), up to 40% of tests may result in false negatives. (False-positive outcomes occur when a test detects cancer when none exists. False-negative outcomes occur when a test finds no malignancy when there is actually cancer present.

Sputum cytology may not be as accurate in detecting other types of lung cancer, especially those that are found in more remote parts of the lungs, than it is in identifying squamous cell lung cancers, which begin in the central airways. Sputum cytology detects less than 50% of cancers that are situated in the periphery (outside areas) of the lungs, but roughly 71% of tumors that are centrally positioned. When sputum cytology yields a negative result (indicating no cancer) in individuals with suspected lung cancer, further testing is advised to definitively establish the presence or absence of lung cancer due to the limitations of sputum cytology.
